Obtaining Information

All community mental health service needs are referred to the ACCESS Committee of Chatham-Kent.  This committee represents the three community-based mental health care providers, CMHA, ACT, and the Mental Health Clinic at the Chatham-Kent Health Alliance.  A physician, psychiatrist, community agency, or family member can make referrals to the ACCESS Committee, or individuals may refer themselves.  Referrals to CMHA come through the ACCESS Committee.

Intake

The CMHA Intake Worker meets with referred client to gather information for the Case Disposition Committee.  The Case Disposition Committee meets weekly to review referrals and decide on the appropriate level of service.  If individuals meet CMHA criteria, they may be accepted for immediate service or to the waiting list or receive brief service.

Service Criteria

CMHA services adults aged sixteen and over experiencing a severe and persistent mental illness who are currently residents of the Municipality of Chatham-Kent or whose family or origin is in Chatham-Kent.

An easy way to remember the admission criteria is to think of the three D's:

Diagnosis - Duration - Disability
